Eleven truths about aligners:

1. Aligners are orthodontic appliances capable of moving teeth as efficiently as braces. A different system of forces is used, but the bone response is the same and should be taken into account in the treatment planning process.

2. Dental technicians do not have the knowledge to correct malocclusions. Clinical orthodontic knowledge is necessary to properly perform aligner treatment.

3. The software for designing aligners and the materials from which they are made are subject to constant development and improvement. The bone response to orthodontic forces is always the same and a good knowledge of bone biology and biomechanics is essential. Using dental techniques and software in isolation from this knowledge will lead nowhere.

4. Treatment with aligners is orthodontic treatment. Occlusion design for aligner treatment requires complete diagnostic data. The doctor is always responsible for the results of treatment. Using the DDP-Ortho software, the treating physician can easily plan and order the production of aligners.

5. A correct description should be sent to the laboratory along with a list of problems and treatment goals. This is a common work standard that requires cooperation with the supplier. The starting material is a digital diagnostic model based on which the final occlusion design is made according to the doctor’s instructions. This description results from the analysis of all diagnostic materials currently available to the clinician (diagnostic model, photo, etc.)

6. Understanding and mastering the biomechanical concepts of tooth movement is a must. Plastic overlays are designed to initiate and carry out specific movements.

7. Tooth movement planning in software must be based on solid biomechanical and biological concepts. The movements simulated by the software may not be possible during treatment if they are not planned correctly.

8. Aligners are not magic. They do not form bone, do not replace orthognathic surgery, and have inherent limitations with tooth movement. All factors must be taken into account before starting aligner treatment.

9. The patient must have realistic expectations regarding treatment.

10. It is very easy to start the treatment process with aligners, but it is much more difficult to complete it to the patient’s satisfaction.  The success of aligner treatment depends on correct case selection, diagnosis and treatment preparation. First, diagnostics in specialized software – then digital design of the patient’s occlusion.

11.Unrealistic expectations often cause patient dissatisfaction and frustration on the part of the treatment provider. The offer must be adapted to the possibilities of implementation.
